Use the links below to download the appropriate set of Accreditation Self-Study Instructions in Microsoft Word format. This format allows you to type directly into the document, using them as a template. It is strongly encouraged you download them in this format if possible. If you do not use Word, you can download the instructions in Adobe PDF format.

Programs should use the self-study instructions for the year during which they will be site visited. This means that accredited programs assigned self-study due dates prior to September 1 should use the current year’s self-study instructions (2013). Accredited programs assigned a self-study due date of September 1 (or later, in the event of a delay), should use the upcoming year’s self-study instructions (2014).

Notes about “eligible” and “accredited, on contingency” applications for internship and postdoctoral residency programs:

  • Application forms for “eligible” status for internship and postdoctoral residency programs will be posted to this page on or about May 1, 2013.

  • Internship and postdoctoral residency programs submitting applications for “Accredited, on contingency” status after September 1, 2013 should use the 2014 CoA Self-study Instructions provided below.

  • Internship and postdoctoral residency programs submitting applications for “Accredited, on contingency” status prior to September 1, 2013 should contact the office at (202) 336-5979.
