All fellowship awards are subject to future funding from the Substance Abuse and Mental Health Services Administration (SAMHSA).

Required Documents to be Submitted Online

  • Brief Cover Letter (no more than 1 page)

  • Application Form

  • Two Essays

  • Substance Abuse Training Plan

  • Curriculum Vita

  • Three Recommendation Letters (with Recommendation Forms)

Required Documents to be Mailed to the MFP Office

  • Official Transcripts

  • GRE Report (institution code 0187; department code 5199 – unofficial report may be uploaded by applicant)

Dates to Remember

  • Application Deadline: January 15

  • Review Process: February and March

  • Notification: early April via mail
