ALL PRE-CONFERENCE WORKSHOPS ARE ON WEDNESDAY, FEBRUARY 20, 2002
Please write the pre-conference and during-conference workshop number(s) you wish to attend on the Registration Form. It is important to enroll early, space is limited. You must enroll specifically to attend pre-conference and during-conference workshops; pre-conference and during-conference workshop fees are NOT included in the general conference registration fees.

To register for pre-conference and during-conference workshops, please write the number of the workshop(s) that you wish to attend on your Conference Registration Form. Please note that you must register for all pre-conference and during-conference workshops. Space is limited. Pre-conference and during-conference workshop fees are NOT included in the registration fees. The Conference Committee reserves the right to cancel any continuing education workshop with insufficient registration.

CE CREDIT GENERAL INFORMATION:
CME Credit
This activity has been planned and implemented in accordance with the Essential Areas and Policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint sponsorship of the American Medical Women's Association and the American Psychological Association. The American Medical Women's Association is accredited by the ACCME to provide continuing medical education for physicians.
The American Medical Women's Association designates this continuing medical education activity for up to 25.45 credit hours in Category 1 towards the Physician's Recognition Award of the American Medical Association. Each physician should only claim those hours of credit that she/he actually spent in each educational activity.
APA CE Credit
This program is co-sponsored by the American Psychological Association and the APA CE Committee. The APA CE Committee is approved by the American Psychological Association to offer continuing education for psychologists. The APA CE Committee maintains responsibility for the program.
MNA CE Credit
Application has been submitted for contact hour approval through the Maryland Nurses Association
( Public Health Education (CECH)
The Society for Public Health Education (SOPHE) will award Continuing Education Contact Hours (CECH) for the conference, including pre-conference and during-conference workshops. SOPHE has been designated a multiple-event provider by the National Commission for Health Education Credentailing.
CANCELLATION/REFUND/SUBSTITUTION POLICY
Registrants who are unable to attend the conference and/or workshop may obtain a refund of their registration fee if a written request is received before February 20, 2002. Full refunds will be given to participants who request a refund on or before February 20, 2002. After that date, a $75.00 handling charge will be assessed for conference cancellation and a $25.00 handling charge for each workshop canceled. If you are unable to attend the conference or workshop(s), a substitution may be made in writing before the conference without charge. "No shows" are subject to the full fee.
In the unlikely event that the conference or any workshop is canceled, full refunds will be given to participants who have registered, but we cannot be responsible for costs that may be incurred for airline and hotel reservations and other expenses.
