For your reference, PDF versions of forms required for publication are provided here.

Based on your submission question responses in Editorial Manager, the journal office will send the appropriate forms to you via Docusign after acceptance. Do not include these in your submission files.

The corresponding author signs all forms on behalf of coauthors. Each coauthor separately completes a questionnaire during submission to confirm their authorship, verify their name for the byline, and disclose any relevant conflict of interest.

The Full Disclosure of Interests Form (121KB)opens in new window must be completed by the corresponding author. Upon submission, all authors are required to disclose via a questionnaire any activities and relationships that, if known to others, might be viewed as a potential conflict of interest.

Permissions Alert Form (244KB)opens in new window
If your paper includes material reprinted or adapted from another source, please review the permissions guidelines in the Permissions Alert Form. Any required permissions and supporting documentation must be obtained and provided to APA staff before publication. After acceptance, all corresponding authors will sign the Permissions Alert Form, attesting that all relevant permissions have been secured. For additional guidance, consult the APA website on copyright and permissions.

Document deposit forms

For authors who wish to use the Green Open Access option when their funding requires that published articles be made publicly available, APA will deposit the final, accepted manuscript to the funder(s) designated repository.

APA currently supports manuscript deposits for the following funders:
