This model displays the four maturity levels (Initial, Emerging, Advancing, and Leading) across the top beginning with the Initial level on the left side and evolving to the Leading level at the other end.

Along the left side, the dimensions (Infrastructure, Accessibility, Knowledge, Engagement, and External-Facing) are listed from top to bottom.

The middle of the model contains high-level descriptions of the activities that will support advancement within the maturity model.

Initial maturity level: Minimal knowledge or use of disability inclusive best practices
Activities: Activities are ad hoc and not well-coordinated

Emerging maturity level: Disability inclusion needs are identified
Activities: Strategic planning initiated and some standardized activities are implemented; emphasis still placed on the impact of individual activities

Advancing maturity level: Disability inclusive practices are implemented
Activities: Plan is fully operationalized including effective and regular use of well-defined policies, processes, and practices; emphasis is on the impact of the work as a whole

Leading maturity level: Serves as an exemplar by fostering an environment where people with ALL types of disabilities feel valued, supported, and included
Activities: Seamless integration of disability inclusion, best practice universal design, and inclusive design; new initiatives are developed and innovative solutions are implemented; still seeks continuous improvement and sustainability
