With the implementation of the Standards of Accreditation (SoA) in 2017, programs are no longer required to submit curriculum vitas for each faculty member. Rather, programs are to fill out a one-page Professional Activities Form for each faculty member who indicates any engagement in professional activities within the past five years in the faculty table(s) completed during the self-study. A blank Professional Activities Form is provided in the self-study instructions under Standard IV.B.1-2 (for doctoral programs), Standard IV.A.2-3 (for internship programs), and Standard IV.B.1 (for postdoctoral programs.
