Documentation—whether written in notes on paper or in an electronic health record is a basic risk management tool, as well as a reflection of a psychologist’s professionalism and credibility. It is also one of the most rushed or overlooked “required tasks.” Documentation is vital to the defense of a licensing complaint or malpractice allegation and may be reviewed by many others, including the patient and potentially a jury.

This presentation reviews basic principles of documentation and how to apply best practices with both the patient and jury in mind.
